Course objectives

The course participants will acquire the knowledge and skills to analyze the problem root causes in order to determine correct and effective solutions. They will learn the basic approach on related to the root cause analysis methods through a structured problem-solving process. Participants will be able to apply appropriate QC Tools and methods to solve the problem and they gain experience in team dynamics.

Course content

• Problem solving, the definition of the term “problem” – naming the problem and problem IS/ IS NOT analysis.
• Attitude towards the problem is, the definition of process, root cause analysis.
• Brainstorming - a principle which uses liberal view of the idea as such and use it in order to define the best ideas to solve the problem.
• 5 x Why – root cause analyse.
• Ishikawa/Fishbone diagram (analyse of possible root causes and their effects related to 6M factors)
• Multivoting (voting within team to define the most important causes in order to achieve consensus)
• Poka-yoke – tool for process mistake proofing systems application
• Basic tools of Quality Control – the commonly used basic QC tools : Pareto analyse, Histogram, Tree diagram, Check sheet, Process Flow, Flow chart
• Continuous Improvement methods and fundamentals: Toyota A3 Report, DMAIC and PDCA cycles
• General scheme of complex problem solving, ownership, accountability, way of thinking, the type of problem
• Problem-solving. Which is the approach in problem-solving? Possible ways to solve the problem.
• The decision-making, selection of options, deciding on following action, planning - preparing for the implementation
• Team exercise for participants: Solving of the simulated tasks.
